Department: Health & Social Services Department

Compensation: $20-$25 based on education and experience
Hours: Part-time, flexible, or on call

ABOUT US
T'ít'q'et Administration is committed to fostering community wellbeing and supporting our most vulnerable community members. Due to an urgent staffing need, we are seeking dedicated and energetic individuals to provide essential Home Care services to our Elders and Persons with Disabilities within the T'ít'q'et community. We have many clients currently requiring immediate support to maintain their independence and dignity in their own homes.
POSITION SUMMARY
Working within the context of the client-centered, family focused vision and trauma informed goals of the T’it’q’et Home and Community Care Program the Home Care Aide combine their, education and experience in health care field supporting client’s care for T’it’q’et First Nation. Home Care Worker assists residents or clients at home with personal hygiene, movement, safety, and nutrition. These positions may also be involved in the delivery of special care to people who are physically or cognitively impaired based on their Care Plans identified by the Home Care Nurse.
KEY RESPONSIBILITIES

  • Following Care Plans identified by Home Care Nurse such activities of daily personal care, bathing, dressing, grooming, and oral hyygiene.

  • Light meal preparation in accordance with designated care plans and supporting the “Meals on Wheels” program.

  • Assistance with mobility and transfers when required.

  • Observing and reporting changes in client behaviour and conditions.

  • Performing delegated tasks according to HCA training and scope of practice

  • Light household duties related to maintaining a safe and healthy environment as needed

  • Reporting to the HCA Team Lead as required.

PREFERRED SKILLS AND EXPERIENCE

  • Following Care Planning and Care Management protocols

  • Understanding chronic disease management, including diabetes, musculoskeletal, dementia spectrum, cardiac and other conditions experience and training.

  • Understanding of wound & ostomy care (Wound care training) would be a benefit

  • Understanding of palliative and End of Life care (pain management) training and experience beneficial

  • Falls and Safety in the Home Assessments for injury prevention and risk reduction

QUALIFICATIONS

  • HCA licensing and registration number required

  • Food Safe Certificate

  • First Aide Certificate

  • WHIMS training
