Department: Health & Social Services Department
Reports to: Health & Social Services Manager
Compensation: $32-$38 based on education and experience
Hours: Part-time, flexible

T'ít'q'et Administration is committed to fostering community wellbeing and supporting our most vulnerable community members. Due to an urgent staffing need, we are seeking dedicated and energetic individuals to provide essential nursing services to our Elders and Persons with Disabilities within the T'ít'q'et community. We have many clients currently requiring immediate support to maintain their independence and dignity in their own homes.
POSITION SUMMARY
Working within the context of the client-centered, family focused vision and trauma informed goals of the T’it’q’et’s Home and Community Care Program and Social Development Adult in Home Care Program, and in accordance with BCCNP Standards of Practice, for Home Care Nurse-LPN combines their health sciences knowledge with assessment, supervisory, and clinical nursing skills to effectively assess and coordinate client care for the T’it’q’et First Nation’s Home and Community Care Programs. The Home Care Nurse-LPN provides direction, guidance and supervision of the Home Support Workers.
KEY RESPONSIBILITIES
- Client assessments, program admission and care plan development and maintenance.
- Home visits and occasional group clinics
- Delegation to and clinical supervision of home care workers and unregulated homemakers providers.
- Consultation, referral, and liaison with other care providers, organizations and institutions.
- Medication management and reconciliation
- Direct trauma-informed, client care for nursing interventions such as wound care, foot care, physical assessment and screening activities.
- Teaching client and family and care providers.
- Partcipate in program review and quality improvement activities based on home care program standards of practice.
PREFERRED SKILLS AND EXPERIENCE
- Assessments, Care Planning and Care Management (Case Management training a benefit)
- Chronic Disease Management, including diabetes, musculoskeletal, dementia spectrum, cardiac and other conditions experience and training.
- Wound & Ostomy Care (Wound care training)
- Palliative and End of Life care (pain management) training and experience beneficial
- Falls and Safety in the Home Assessments for injury prevention and risk reduction
- Supervision and adult learning principles
- Recent acute care nursing skills are beneficial.
QUALIFICATIONS
- Nurse licensing and registration number required
