Position Summary

The Lillooet Medical Clinic is seeking a Permanent Full Time Nurse Practitioner Interior Health is seeking a dynamic Nurse Practitioner to join the Lillooet/Lytton Collaborative Care Model, providing comprehensive primary and urgent care to residents of Lytton, Lillooet, surrounding rural communities, and the Nlaka’pamux and St’át’imc Nations.

Working as part of an interdisciplinary team, the Nurse Practitioner will provide full-scope, culturally safe and patient-centred care across a flexible rural model that includes longitudinal primary care, urgent and episodic care through the Lytton Urgent and Primary Care Centre (UPCC), which provides services seven days per week, virtual care, and community outreach. NPs may serve as Most Responsible Provider (MRP) for an attached patient panel, supporting continuity of care across settings, including ongoing care for their attached patients who transition to Long-Term Care.

The NP will work collaboratively with family physicians, nursing, Mental Health and Substance Use, Home and Community Care, Indigenous Health, allied health professionals, and community partners. Care may be provided in person, virtually, and through planned outreach to surrounding communities, supporting improved access for people living in rural and remote areas.

This is an exciting opportunity to practice to full NP scope within an evolving, team-based rural care model that values autonomous practice, collaboration, cultural safety, continuity, and strong relationships with the communities served. Participation in the NP/Physician Long-Term Care On-Call Program may be required as part of the role.

Salary Range: Salary range for the position is $76.61/hour - $84.27/hour. Interior Health establishes salaries within the minimum and maximum of the salary range based on consideration of the qualifications, experience of the applicant.

Key Responsibilities:

Diagnosis and Treatment: Assess and diagnose undifferentiated diseases and conditions, provide treatment, and prescribe medications in line with the Nurse Practitioner’s scope of practice and regulatory standards.

Care Management: Monitor ongoing care, order diagnostic investigations, interpret results, and develop tailored treatment plans to optimize patient outcomes.

Patient Communication: Establish care priorities, provide follow-up treatment, and communicate effectively with patients and families about diagnoses, care plans, and prognoses.

Collaboration and Consultation: Work closely with physicians and other healthcare professionals to assess, diagnose, and create individualized treatment plans. May also admit and discharge patients as per organizational policies.

Prescribing Medication: Prescribe medications within legal and regulatory guidelines, ensuring the safe and effective use of pharmaceutical treatments.

Leadership and Supervision: Lead and guide other nursing and healthcare personnel. Provide education, supervision, and performance evaluations to support professional development.

Research and Innovation: Contribute to research aimed at enhancing patient care and advancing nursing practices. Participate in initiatives to influence healthcare policies and population health strategies.

Health Promotion: Lead health promotion activities and screenings, focusing on at-risk populations to prevent illness and promote well-being.

Education and Mentorship: Facilitate interdisciplinary education through case presentations, mentoring, and collaboration in clinical and community settings. Support continuous learning and knowledge sharing.

Policy Development: Develop, implement, and evaluate policies related to nursing, interdisciplinary care, and health system practices.

Palliative and Long-Term Care: Serve as the most responsible provider for patients in palliative, hospice, and long-term care settings when required.

Safety and Compliance: Adhere to Occupational Health and Safety guidelines to maintain a safe and healthy work environment. Ensure compliance with training and incident investigation protocols.

Additional Responsibilities: Perform other duties as necessary to support patient care and organizational goals.

Qualifications

Education: Master’s degree in Nursing (NP) or equivalent.
Certification: Current Nurse Practitioner certification with the British Columbia College of Nurses and Midwives
Experience: Previous experience in a clinical or healthcare setting, with the ability to diagnose, treat, and manage patient care is considered an asset.
